Description
Running along the apices of the lumbar spinous processes, the supraspinous ligament appears as a thick, cord-like band in the posterior midline, bridging from one vertebra to the next. Inferiorly it approaches the sacrum at the lumbosacral junction, while superiorly it continues toward the thoracolumbar region where the deep contour of the rib cage begins to enter the field. Lateral to this midline ligament, the iliac crest and posterior ilium frame the lower spine, and the acetabulum with the proximal femur sits anterolateral, giving clear bony context for pelvic orientation. Clinically, the supraspinous ligament is one of the stabilizing restraints that limits flexion in the lumbar spine, and it is a frequent pain generator in interspinous or supraspinous sprain after sudden trunk flexion or lifting injuries. Palpation tenderness over the spinous tips can localize posterior ligamentous injury, and MRI may show edema between the spinous processes when the posterior ligament complex is involved. Extension into the upper spine links it conceptually to the ligamentum nuchae, a helpful teaching bridge when tracing posterior midline ligaments from cervical to lumbar regions. Course directors can drop this figure into gross anatomy or musculoskeletal modules when explaining posterior spinal ligaments, lumbar stability, and the relationship between vertebral landmarks and pelvic anatomy.
