Description
Oriented in a true lateral profile of the male crus, the illustration places the tibia anteromedially and the fibula posterolaterally, with the anterior compartment muscles (tibialis anterior and extensor digitorum longus) running inferiorly toward the dorsum of the foot. Along the lateral surface, fibularis (peroneus) longus and fibularis brevis form a distinct band over the fibular shaft, their tendons coursing posterior to the lateral malleolus under the superior and inferior fibular retinacula. Posteriorly, the lateral head of gastrocnemius blends with soleus to form the calcaneal (Achilles) tendon inserting on the posterior calcaneal tuberosity, while the ankle and midfoot bones (talus, calcaneus, cuboid, metatarsals, and phalanges) provide clear distal landmarks. Tendon trajectories are easy to follow. Lateral lower-leg anatomy matters because it is where clinicians localize pain from peroneal tendinopathy, subluxation of the fibularis brevis tendon, and lateral ankle sprain patterns involving the anterior talofibular and calcaneofibular ligaments. The relationship between the fibularis tendons and the lateral malleolus explains why swelling after inversion injury can mimic a tendon problem and why retinacular disruption can lead to snapping. Compartment boundaries also read well here. That matters in exertional compartment syndrome.
