Description
Running from the distal femur through the knee into the crus and foot, the tibia lies medial and the fibula lateral, framing the interosseous membrane and the muscle compartments that wrap the leg. Anteriorly, extensor groups track toward the dorsum of the foot, while the posterior compartment thickens into the triceps surae, converging distally into the calcaneal (Achilles) tendon inserting on the posterior calcaneus. Around the ankle, long tendons funnel in tight corridors behind the malleoli, with retinacula binding them close to bone as they pass into the tarsals, metatarsals, and phalanges. A key landmark. The distal lower leg is where anatomy becomes crowded and pathology becomes predictable: limited space, high loads, and sharp turns around bony pulleys. Clinicians care about the exact order of structures posterior to the medial malleolus (tibialis posterior, flexor digitorum longus, posterior tibial vessels, tibial nerve, flexor hallucis longus) when evaluating tarsal tunnel syndrome or posterior tibial tendon dysfunction, and about the peroneus longus and brevis tendons as they course posterior to the lateral malleolus in chronic ankle instability. The posterior view of the heel also anchors discussion of Achilles tendinopathy and rupture, including the watershed zone 2 to 6 cm proximal to the calcaneal insertion and the relationship of the tendon to the retrocalcaneal bursa. These are common exam and operative waypoints.
