Description
Shown in lateral profile, the major alar cartilage (greater alar cartilage) forms the anterior and inferior framework of the external nose, curving around the apex to shape the margin of the naris. Its medial crus lies adjacent to the caudal nasal septum and anterior nasal spine, while the lateral crus extends posterolaterally over the maxilla toward the alar base. Surrounding bony landmarks of the viscerocranium are also visible, including the nasal bone superiorly, maxilla anteriorly and inferiorly, zygomatic bone laterally, and mandible inferiorly, with dentition in occlusion. Cartilaginous components of the nasal cavity, including the septal cartilage and conchae within the nasal aperture, provide internal context. Lateral orientation matters when you need to teach or plan how alar support maintains the external nasal valve and the contour of the ala during inspiration. Even small changes in the shape or stiffness of the lateral crus, whether from trauma, prior rhinoplasty, or congenital asymmetry, can narrow the vestibule and produce dynamic collapse that patients describe as unilateral obstruction. This is the view that clarifies where alar batten grafts or lateral crural strut grafts are placed relative to the piriform aperture and the caudal septum. A critical landmark.
