Description
Positioned deep along the posterior cervical column, the medial semispinalis capitis ascends from the cervical and upper thoracic transverse processes to the occipital bone, inserting between the superior and inferior nuchal lines. From a lateral perspective, its fibers course superiorly and slightly medially, lying posterior to the cervical vertebral arches and just medial to the more superficial splenius capitis and upper trapezius when those layers are conceptually removed. Superiorly, the muscle approaches the mastoid region but remains centered toward the midline compared with sternocleidomastoid, which sits more anterior and lateral as it runs from the mastoid process to the manubrium and clavicle. Bony landmarks such as the occiput, atlas and axis, and the spinous processes of the mid to lower cervical vertebrae provide the orientation. Semispinalis capitis is a primary extensor of the head and a strong contralateral rotator, so its line of pull becomes obvious in a clean side view that preserves the occipital attachment and the posterior elements of the cervical spine. This is headache anatomy. Myofascial trigger points in semispinalis capitis can refer pain to the vertex and occipital region, and the muscle’s proximity to the greater occipital nerve as it emerges near the superior nuchal line helps explain why posterior neck overuse, whiplash injury, or postoperative spasm after posterior cervical approaches can present with occipital neuralgia-like symptoms.
