Description
Running vertically along the posterior cervical spine, the medial semispinalis capitis is highlighted against a full body male skeletal backdrop, with fibers ascending from the cervical and upper thoracic transverse processes toward the occipital bone between the superior and inferior nuchal lines. Medially, the muscle hugs the spinous processes and laminae, blending visually with deeper transversospinalis layers such as multifidus and the semispinalis cervicis, while lying deep to the trapezius and splenius capitis in the upper neck. Superiorly it approaches the midline occiput, and inferiorly it tapers toward the lower cervical region, framed laterally by the scapulae and posterior shoulder girdle. Bone landmarks remain clear: skull, vertebral column, ribs, pelvis, and long bones of the limbs in posterior anatomical position. Posterior neck pain is rarely just “neck pain,” and semispinalis capitis often sits at the center of it. Hypertonicity or strain after whiplash can refer pain to the occiput and vertex, mimicking cervicogenic headache, and trigger points in the medial belly are a common finding in patients with sustained extension postures. For posterior cervical approaches, this muscle defines a layer you must split or reflect to reach the laminae, and it is innervated by the dorsal rami, a useful teaching point when correlating symptoms with segmental anatomy. Use this artwork in gross anatomy and kinesiology teaching to contrast superficial trapezius with the deeper transversospinalis group, or in clinical education materials on cervicogenic headache, whiplash-associated disorders, and posterior cervical decompression technique.
