Description
Running along the lateral border of the male forearm, the superficial extensor-supinator mass is arranged from anterior to posterior around the radius, with brachioradialis forming a prominent anterolateral column from the lateral supracondylar ridge toward the distal radius near the radial styloid. Posterior to it, extensor carpi radialis longus and brevis track distally toward the bases of the second and third metacarpals, while extensor digitorum occupies a more posterior, central position and becomes tendinous as it approaches the dorsum of the hand. Proximally, the muscle bellies crowd the lateral epicondylar region, then narrow into tendons as they pass deep to the extensor retinaculum at the wrist. One muscle is color-emphasized in red for quick localization against the surrounding fibers. Lateral forearm anatomy matters because this is where common overuse pain localizes and where clinicians palpate to separate radial-sided tendinopathy from nerve entrapment. Lateral epicondylitis centers on the extensor carpi radialis brevis origin at the lateral epicondyle, while pain that tracks distally into the proximal dorsolateral forearm raises concern for radial tunnel syndrome involving the posterior interosseous nerve as it courses through the supinator. Small differences in tendon trajectory at the radial styloid help explain why first dorsal compartment stenosing tenosynovitis (De Quervain disease) presents with focal tenderness and crepitus at the radial wrist.
