Description
Seen in true sagittal section from a lateral perspective, the cerebral hemispheres dominate superiorly with cortical gyri and sulci leading into the medial wall, where the corpus callosum arches in a characteristic C shape above the lateral ventricle. Inferior to the callosal body, the diencephalon sits centrally, with the thalamus positioned superior and posterior to the hypothalamus, which forms the floor of the third ventricle and continues inferiorly toward the infundibular region. Posterior and inferior to the cerebrum, the cerebellum occupies the posterior cranial fossa, while the brainstem (midbrain, pons, medulla oblongata) forms a ventral, caudally directed column connecting intracranial structures to the cervical spinal cord. Clean midline anatomy. This cut is the workhorse for teaching ventricular relationships and for orienting cross sectional neuroimaging: the corpus callosum, third ventricle region, and the stepwise transition from midbrain to pons to medulla map directly onto routine sagittal MRI. Clinically, the view helps localize mass effect at the midline, such as transtentorial (uncal) herniation with brainstem compression, and it provides the reference anatomy used when describing hydrocephalus, colloid cyst obstruction near the foramen of Monro, or brainstem infarcts affecting medullary respiratory centers. Neurosurgical corridors are planned from this geometry.
