Presented in lateral profile with a midline sagittal section, the cerebrum dominates superiorly and anteriorly, its cortical gyri and sulci defining the frontal, parietal, temporal, and occipital lobes around the central sulcus, lateral (Sylvian) fissure, and parieto-occipital sulcus. Deep to the cingulate gyrus, the corpus callosum arches anteroposteriorly, with the septum pellucidum and the lateral ventricle situated inferior to its body and superior to the thalamus. Posterior and inferior, the cerebellum sits behind the brainstem, where the arbor vitae is exposed, while the midbrain, pons, and medulla oblongata descend toward the foramen magnum. A sagittal section is the cleanest way to teach the relationship between the ventricular system and the diencephalon, and it is also the view clinicians default to when localizing pathology along the midline. Hydrocephalus, for example, is often discussed in terms of third ventricle enlargement and aqueductal narrowing, and this perspective keeps the corpus callosum, fornix region, thalamus, and hypothalamus in one continuous anatomic line. Midline shift is not subtle here.
