Description
Seen from a lateral shoulder perspective in an adult male, the suprascapular nerve is traced from its origin at the upper trunk of the brachial plexus (predominantly C5 to C6) as it courses posterolaterally toward the superior border of the scapula. The nerve approaches the suprascapular notch at the root of the coracoid process, passing deep to the superior transverse scapular ligament while the suprascapular artery typically travels superficial to that ligament. Distally, it enters the supraspinous fossa to supply supraspinatus, then curves around the lateral edge of the scapular spine through the spinoglenoid notch to reach infraspinatus. Clear landmarks. That relationship between the suprascapular notch, the superior transverse scapular ligament, and the adjacent coracoid base is the anatomic setup for suprascapular neuropathy, classically from entrapment at the notch or traction in overhead athletes. Compression at the suprascapular notch can denervate both supraspinatus and infraspinatus, while a lesion at the spinoglenoid notch preferentially affects infraspinatus, a distinction that matters when you correlate shoulder pain and weakness with EMG findings and MRI muscle atrophy patterns. Surgeons also use these landmarks during arthroscopic decompression or when navigating posterior portals near the glenoid and scapular spine. Use this lateral model view for teaching brachial plexus branch anatomy in gross anatomy and kinesiology courses, for labeling plates in orthopedic or sports medicine texts, and for clinical education on posterior shoulder pain, rotator cuff weakness patterns, and nerve block planning around the suprascapular notch.
