Description
Arising from the superior trunk of the brachial plexus (C5 to C6), the suprascapular nerve runs laterally across the posterior triangle of the neck toward the superior border of the scapula. From this lateral perspective, its course relates closely to the clavicle and first rib anteriorly and the cervical spine medially, then tracks toward the suprascapular notch at the root of the coracoid process. The scapula sits posterior and lateral to the rib cage, and the nerve’s path is shown as it approaches the suprascapular ligament region before entering the supraspinous fossa. Clear bony landmarks anchor the pathway. Clinically, the suprascapular notch is where the nerve is classically compressed, often beneath the transverse scapular ligament, producing posterior shoulder pain and weakness of supraspinatus and infraspinatus with loss of abduction initiation and external rotation strength. A lateral view helps clarify why traction across the neck, bulky ganglion cysts at the spinoglenoid notch, or overhead sports mechanics can irritate this nerve while sparing adjacent trunks and cords of the brachial plexus. This is a nerve you palpate around indirectly, but you decompress and block with anatomy in mind.
