Description
Seen from the lateral aspect of the left cerebral hemisphere, the superior temporal gyrus forms a long cortical ridge on the inferolateral surface of the temporal lobe, running posteriorly beneath the lateral sulcus (Sylvian fissure). Superiorly it is bounded by the lateral sulcus, while inferiorly it is separated from the middle temporal gyrus by the superior temporal sulcus; the anterior pole of the temporal lobe lies rostral to these landmarks. A blue highlight localizes the hippocampus deep to the temporal neocortex, inferomedial to the temporal horn region and anterior to the parahippocampal gyrus, with the brainstem and its continuity toward the spinal cord visible on the basal aspect. Clear sulci and gyri define the lateral convexity. Orientation is immediate. This lateral perspective matters because the superior temporal gyrus is the surface gateway to perisylvian language and auditory cortex, and its posterior portion neighbors Wernicke area on the dominant hemisphere. Stroke, tumor, or cortical dysplasia in this region classically produces receptive aphasia or auditory processing deficits, while the deep hippocampal emphasis connects the same temporal lobe territory to mesial temporal sclerosis and focal impaired-awareness seizures. The juxtaposition of neocortex and hippocampus also frames why anterior temporal lobectomy and selective amygdalohippocampectomy require careful respect for lateral temporal language cortex and deep white matter pathways.
