Description
Posteriorly oriented, the superior temporal gyrus (gyrus temporalis superior) is emphasized on the lateral aspect of each cerebral hemisphere, tracking anteroposteriorly along the superior margin of the temporal lobe and bordering the lateral sulcus superiorly. From this angle the posterior segment approaches the temporoparietal junction, with the supramarginal and angular gyri lying superior and posterior to it across the posterior end of the Sylvian fissure. The cortical surface relief is clear, with adjacent sulci defining the convolutional anatomy, and the cerebellum sits inferior to the occipital poles in the posterior cranial fossa. Bilateral symmetry is easy to judge. This posterior emphasis on the superior temporal gyrus matters because its posterior third, on the dominant side, corresponds to the classic receptive language territory around Wernicke cortex and the posterior superior temporal plane, while the right homolog more often supports prosody and complex auditory scene analysis. Lesions in this region, whether from an inferior division middle cerebral artery infarct, contusion near the temporoparietal region, or low-grade glioma, can produce fluent aphasia with impaired comprehension, alexia, or auditory verbal agnosia depending on extent and lateralization. The proximity to the posterior insula and peri-Sylvian cortex also makes it a practical landmark when teaching perisylvian language mapping and planning awake craniotomy trajectories that minimize postoperative language morbidity.
