Description
Running longitudinally along the anterior aspects of the vertebral bodies, the anterior longitudinal ligament (ALL) forms a continuous band from the inferior cervical spine across the thoracic and lumbar regions to the anterior surface of the sacrum. A lateral perspective places the ligament immediately anterior to the intervertebral discs and vertebral body margins, tracking the normal cervical lordosis, thoracic kyphosis, and lumbar lordosis in profile. Superiorly, the skull and upper cervical vertebrae provide cranial context, while inferiorly the sacrum, coccyx, and pelvic ring frame the lumbosacral junction where the ALL broadens. Bone is the backdrop. Clinically, the ALL is the primary restraint to spinal extension and a key landmark in anterior approaches to the cervical and lumbar spine, where the surgeon works in the prevertebral or retroperitoneal plane before reaching the anterior disc space. This view clarifies why osteophytes from cervical spondylosis and ossification of the anterior longitudinal ligament (a component of diffuse idiopathic skeletal hyperostosis) project anteriorly and can contribute to dysphagia when bridging occurs along the anterolateral vertebral bodies. It also helps explain why hyperextension injury patterns differ from flexion injuries, given the ligament’s broad, firm attachment to the vertebral body periosteum compared with its looser relationship to the disc.
