Description
Spanning from the anterior superior iliac spine to the pubic tubercle, the inguinal ligament forms the inferior, rolled margin of the external oblique aponeurosis along the anteroinferior abdominal wall. Medial to the ilium and lateral to the pubic symphysis, it arches over the iliopsoas and pectineus to define the floor of the inguinal canal and the superior boundary of the femoral region. An anterior bony framework accompanies it, including the iliac crest and ala, superior and inferior pubic rami, ischial components, and the proximal femora seated in the acetabula, with the sacrum, coccyx, and lower lumbar vertebrae providing posterior midline context. Clear landmarks. For hernia anatomy, this relationship matters because the inguinal ligament is the reference line for distinguishing inguinal hernias (superior and medial to the ligament) from femoral hernias (inferior to the ligament, at the femoral canal). Surgeons also track it to orient an open inguinal approach and mesh placement, where the ligament and adjacent pubic tubercle serve as fixation landmarks, and to avoid injuring the ilioinguinal and genitofemoral nerves as they traverse the region and emerge near the superficial inguinal ring. Teaching teams often pair this anterior view with cross-sectional imaging to help learners translate surface landmarks into the underlying osteology and fascial planes of the groin. It also fits well in general surgery texts, anatomy lab manuals, and patient-facing diagrams explaining groin pain and suspected hernia location relative to the pubis and iliac spine.
