Description
Anteriorly oriented anatomy of the male pelvis is presented with the sacrum centered between the right and left os coxae, and the lumbar vertebrae (L1 to L5) stacked superiorly. Along the ventral aspect of each sacroiliac joint, the anterior sacroiliac ligament appears as numerous thin, fanlike fibers spanning from the auricular surface and ala of the sacrum to the adjacent iliac tuberosity region, lying deep to the pelvic fascia. Inferiorly, the coccyx continues from the sacral apex, while the acetabula sit lateral to the joint line, with the proximal femoral heads positioned inferolateral to the ilia. Because the anterior sacroiliac ligament is thin compared with the interosseous and posterior sacroiliac ligaments, an anterior perspective helps clarify what it can and cannot restrain at the sacroiliac articulation. Its fiber direction relates directly to anterior gapping and shear mechanics that become clinically relevant in pelvic ring trauma, postpartum pelvic girdle pain, and suspected sacroiliac joint dysfunction. It also matters in anterior approaches to the pelvis and lumbosacral region, where dissection planes pass near the ventral sacroiliac complex and misidentification can lead to an inaccurate understanding of joint stability. Orthopaedic and sports medicine texts can pair this view with posterior ligament plates to teach why most sacroiliac stability is posterior, while still accounting for ventral capsuloligamentous support.
