Description
Seen from a lateral perspective in an adult male, the posterior cervical musculature is oriented from the upper thoracic and lower cervical spinous processes toward the occiput, with semispinalis capitis coursing superiorly and slightly anteriorly to insert between the superior and inferior nuchal lines. Deep to more superficial layers such as splenius capitis and trapezius, its fibers lie posterior to the cervical vertebral column and posterior to the sternocleidomastoid, which runs obliquely from the manubrium and clavicle to the mastoid process. Bony landmarks expected in this view include the occipital bone, mastoid region of the temporal bone, cervical vertebrae, upper ribs, clavicle, and scapula. Clear nuchal attachments. Semispinalis capitis matters because it is a primary extensor of the head and a strong contributor to contralateral rotation, and its segmental innervation via the dorsal rami makes it a key reference when teaching posterior neck anatomy and motor control. Clinically, this muscle is encountered during posterior cervical approaches, where midline dissection and subperiosteal elevation expose the deep extensors near the spinous processes, and it also serves as a common generator of cervicogenic headache patterns with tenderness near the occipital attachments. The lateral relationship to sternocleidomastoid helps clarify why anterior neck pain and posterior suboccipital pain can be mislocalized without a firm mental map.
