Splenius Capitis Muscle

Description

Arising from the inferior half of the ligamentum nuchae and the spinous processes of C7 to T3 or T4, the splenius capitis forms a flat, obliquely oriented sheet on the posterolateral neck in the adult male. Its fibers run superolaterally, passing deep to the trapezius and sternocleidomastoid, to insert on the lateral third of the superior nuchal line of the occipital bone and the mastoid process of the temporal bone. Medially it borders the midline cervical spinous processes, while laterally it helps define the muscular contour that overlies the posterior triangle. Deep to it lie the semispinalis capitis and longissimus capitis, layered in the classic extrinsic to intrinsic sequence. Orientation of the splenius capitis matters because its oblique fiber direction makes it a primary landmark for differentiating posterior cervical muscle layers during dissection and in posterior neck pain workups. Unilateral contraction produces ipsilateral rotation and lateral flexion, a pattern that can mimic facet-mediated pain or cervicogenic headache when myofascial trigger points refer toward the mastoid and occiput. For posterior approaches to the upper cervical spine, clean separation of splenius capitis from semispinalis capitis helps preserve the deeper suboccipital region and reduces unnecessary muscle stripping.
