Description
Shown in lateral profile, the splenius cervicis is highlighted deep in the posterior neck, lying posterior to the cervical transverse processes and running obliquely superolaterally from the midline thoracic spinous processes toward the upper cervical vertebrae. Its fibers typically arise from the spinous processes of T3 to T6 and insert onto the transverse processes of C1 to C3 (often extending to C4), placing it deep to trapezius and splenius capitis while remaining superficial to the deeper transversospinalis layer, including semispinalis cervicis and the medial spinalis. Anteriorly, sternocleidomastoid and the scalene muscles frame the neck, while inferiorly the muscle blends into the posterior triangle region above the shoulder girdle. Functionally, splenius cervicis acts with its ipsilateral partner to extend the cervical spine and with unilateral contraction to laterally flex and rotate the neck to the same side, movements commonly limited after whiplash-associated disorders and posterior cervical muscle strain. Pain referral from posterior cervical trigger points can track into the lower occiput and lateral neck, and its proximity to the dorsal rami makes this layer relevant when planning medial branch blocks, radiofrequency ablation targets, or interpreting paraspinal EMG findings. A clean lateral view also clarifies the plane surgeons develop during posterior cervical approaches, where preserving the superficial layer can reduce postoperative neck pain.
