Description
Positioned superior to the patella and deep to the quadriceps femoris tendon, the suprapatellar synovial bursa is shown in lateral profile as a thin-walled synovial sac that expands into the suprapatellar recess of the knee joint. The distal femur forms the posterior boundary, with the anterior contour defined by the extensor mechanism as it courses toward the tibial tuberosity. Inferiorly, the bursa communicates with the synovial cavity between the femoral trochlea and the region where the patella would normally ride, even if the patella itself is not rendered in this side view. Depth is the message. A lateral angle clarifies how the suprapatellar bursa accommodates excursion of the quadriceps tendon during flexion and extension and why fluid preferentially accumulates there when the knee is distended. Clinically, suprapatellar effusion is a common finding in inflammatory synovitis, post-traumatic hemarthrosis, and septic arthritis, and this recess is the target for aspiration or injection when the patellofemoral space is difficult to access. The relationship to the anterior distal femur also helps distinguish true intra-articular effusion from prepatellar bursitis, which sits superficial to the patella and patellar tendon rather than deep to the extensor apparatus.
