Description
Presented from an anterior perspective, the shoulder complex is oriented around the proximal humerus as it meets the glenoid cavity of the scapula to form the glenohumeral joint. Superiorly, the acromion process overhangs the humeral head, with the lateral clavicle approaching the acromioclavicular region, while segments of the upper ribs provide thoracic context medially and inferiorly. Between the undersurface of the acromion and the rotator cuff footprint over the greater tubercle lies the subacromial (subdeltoid) synovial bursa, positioned as a flattened sac that reduces friction during elevation and abduction of the arm. Anterior visualization of the subacromial bursa matters when teaching the mechanics of painful arc and subacromial impingement, where bursal inflammation accompanies supraspinatus tendinopathy and narrowing beneath the coracoacromial arch. It is also the anatomic target for ultrasound guided subacromial bursal injection, where needle trajectory is planned to remain superficial to the rotator cuff tendons and deep to the deltoid, avoiding inadvertent intratendinous placement.
