Description
Anteriorly, the synovial bursa overlying the rotator cuff is presented as a distended sac interposed between the deltoid region and the superior aspect of the proximal humerus. Deep to it sit the humeral head and anatomical neck, articulating with the glenoid cavity of the scapula, while the clavicle tracks medially toward the acromial region and the lateral ribs form the thoracic backdrop. The bursal contour follows the subacromial and subdeltoid spaces, draping superior to the greater tubercle where supraspinatus tendon fibers would course toward their insertion. Clear relationships. Clinical teaching often treats the subacromial bursa as an afterthought, yet this anterior perspective makes its friction-reducing role under the acromion and coracoacromial arch easy to explain when discussing painful arc and impingement syndromes. Bursal thickening and fluid, whether from overuse, calcific tendinopathy, or rotator cuff tearing, commonly present with anterolateral shoulder pain and are frequently confirmed on ultrasound by dynamic assessment during abduction. The same anatomy guides subacromial corticosteroid injection, where needle placement aims for the bursal plane superficial to supraspinatus and deep to deltoid, avoiding the glenohumeral joint capsule. Orthopedic modules and gross anatomy labs can pair this plate with rotator cuff tendon diagrams to differentiate extra-articular bursal pathology from intra-articular glenohumeral disease. It also suits radiology teaching files as a companion to anterior ultrasound and coronal oblique MRI sequences that demonstrate bursal distension relative to the humeral head and acromial undersurface.
