Seen in lateral profile, the T7 vertebra presents a heart shaped vertebral body anteriorly, with the pedicles and laminae sweeping posteriorly to complete the vertebral arch around the vertebral foramen. Superior and inferior articular processes project at the junction of pedicle and lamina, their zygapophyseal facets oriented to favor rotation while limiting flexion and extension in the mid thoracic spine. A long, inferiorly sloping spinous process extends posteriorly, and the transverse process projects laterally, bearing the transverse costal facet for articulation with the tubercle of the seventh rib. This is the rib joint. That transverse costovertebral relationship is the teaching point here, because it is where thoracic cage mechanics meet spinal kinematics: the costotransverse joint at T7 participates in the pump handle and bucket handle motion of respiration and becomes a pain generator in costovertebral or costotransverse joint dysfunction.
