Description

Seen in lateral profile, the male thoracic vertebrae T1 through T12 stack superior to inferior to form the normal posterior convexity of the thoracic kyphosis. Vertebral bodies lie anteriorly with intervertebral discs interposed, while the spinous processes project posteriorly and angle inferiorly, overlapping more in the mid thoracic levels. Transverse processes extend laterally from each vertebral arch, and the superior and inferior articular processes define the zygapophysial (facet) joints that guide rotation and limit flexion and extension. Thoracic alignment matters because it sets the mechanical relationship between the cervical and lumbar curves and because deformity here changes rib cage volume and scapulothoracic mechanics. A true lateral view is the reference for measuring kyphosis (commonly with a Cobb angle from T4 to T12) and for distinguishing physiologic rounding from hyperkyphosis due to Scheuermann disease, vertebral compression fracture, or ankylosing spondylitis. It also maps levels for thoracic epidural and paravertebral blocks and helps communicate the expected inferior slant of spinous processes when selecting a midline approach. Orientation is everything.
