Description
Medial anatomy of the posterior leg is arranged around the triceps surae complex, with the gastrocnemius and soleus converging into the calcaneal (Achilles) tendon as it descends posterior to the distal tibia and inserts on the posterior surface of the calcaneal tuberosity. From this perspective the tibial shaft and medial contour of the ankle frame the tendon anteriorly, while the heel pad and posterior calcaneus sit inferior to the insertion site. Proximally, the tendon lies superficial to the deep posterior compartment and blends with the crural fascia, and distally its fibers broaden slightly as they approach the calcaneus. A clean medial silhouette. Clinically, a medial view clarifies the relationship between the Achilles tendon and the calcaneus that underlies insertional tendinopathy, retrocalcaneal bursitis, and Haglund-type posterior calcaneal prominence, where friction and compressive load concentrate at the distal 2 to 3 cm. Acute rupture most often occurs 2 to 6 cm proximal to the insertion in the watershed zone of relatively lower vascularity, a key teaching point when correlating pain location with palpation gaps, Thompson test findings, and operative versus nonoperative management. Medial exposure is also a common surgical corridor for debridement and calcaneal exostectomy, chosen to reduce risk to the sural nerve, which courses more laterally.
