Description
Running along the posterior aspect of the distal leg, the calcaneal (Achilles) tendon is rendered as a thick cord formed by the gastrocnemius and soleus (triceps surae) and descending to its insertion on the posterior calcaneal tuberosity. From this lateral perspective, the fibula lies lateral to the tibia, with the lateral malleolus projecting distally to frame the talocrural (ankle) joint over the talus. The calcaneus sits inferior and posterior to the talus, and the midfoot and forefoot bones extend anteriorly into the metatarsals and phalanges; proximally, the distal femur, tibial plateau, and patella establish the knee reference. Fibularis longus is visible on the lateral compartment, its muscle belly posterior to the fibula proximally and its tendon course implied toward the plantar foot. Lateral viewing matters for teaching how the Achilles tendon relates to the lateral malleolus, subtalar region, and the posterior calcaneus, where insertional tendinopathy and Haglund-related irritation commonly localize. Rupture often occurs 2 to 6 cm proximal to the calcaneal insertion, a segment with limited vascularity, and this side view clarifies the typical gap location relative to the heel and the distal leg. Palpation and surgical planning also benefit from recognizing nearby lateral soft-tissue corridors, including the expected course of the sural nerve and small saphenous vein posterolaterally.
