Description
Seen from the medial aspect of the male lower leg, the small plantaris muscle belly arises near the lateral supracondylar line of the femur and the oblique popliteal ligament, then quickly tapers into a long, slender tendon that descends inferomedially. Proximally it sits deep to the gastrocnemius and superficial to the popliteus region, while distally its tendon tracks between gastrocnemius and soleus toward the posteromedial calcaneus, running adjacent to but typically separate from the calcaneal (Achilles) tendon. The tibia lies medial and the fibula lateral, with the knee and ankle joints anchoring the proximal and distal relationships of the posterior compartment musculature. Small by volume, easy to miss. This medial perspective matters because the plantaris tendon often mimics pathology of the Achilles on exam and imaging, yet its injuries behave differently. A classic scenario is so-called tennis leg, where rupture of the plantaris tendon or a tear at the medial head of gastrocnemius produces acute posteromedial calf pain and swelling, sometimes raising concern for deep vein thrombosis. The slender tendon also becomes a surgical consideration: it is frequently harvested as an autograft for hand tendon reconstruction or ligament procedures, and surgeons rely on its course relative to the medial gastrocnemius, soleus, and calcaneal tendon to avoid iatrogenic injury during posterior leg or hindfoot approaches.
