Description
Arising from the lateral supracondylar line of the femur and the adjacent oblique popliteal ligament, the plantaris belly sits posterior to the knee, just superior to the medial head of gastrocnemius and superficial to the popliteus. Its slender tendon quickly becomes conspicuous on a medial exposure, coursing distally between gastrocnemius and soleus, then tracking along the posteromedial tibia toward the calcaneus. Inferiorly, the plantaris tendon lies medial to the calcaneal (Achilles) tendon or blends with its medial margin near the posterior calcaneus. Small muscle. Long tendon. This medial perspective matters because plantaris injury commonly presents as the so-called tennis leg, where a sudden forced dorsiflexion with knee extension can rupture the plantaris or the medial gastrocnemius aponeurosis and mimic deep vein thrombosis with acute calf pain and swelling. Surgeons also encounter the tendon during posteromedial approaches to the knee and ankle, and it is frequently harvested as an autograft for hand tendon reconstruction, so its relationship to the gastrocnemius-soleus interval and the posterior compartment fascia becomes practical anatomy. The wispy morphology in a male calf also helps teach why plantaris is often absent or variable, and why its tendon can be mistaken for a split portion of the Achilles on imaging. Use this artwork in lower limb anatomy labs, sports medicine teaching files on calf strain differentials, or operative atlases that describe graft harvest and posteromedial dissection planes around the knee and hindfoot.
