Description
Running along the plantar-medial forefoot, the flexor hallucis brevis (flexor hallucis brevis muscle of the foot) appears as two stout bellies that flank the flexor hallucis longus tendon as it courses distally toward the base of the proximal phalanx of the hallux. Medially, the muscle sits deep to the plantar aponeurosis and in close relationship to the abductor hallucis, while its distal tendinous slips bracket the first metatarsophalangeal joint and associate with the medial and lateral sesamoid apparatus. Proximal bony landmarks include the calcaneus and talus, with the navicular and medial cuneiform supporting the medial longitudinal arch as the first metatarsal projects anteriorly. Functionally, this view clarifies how the flexor hallucis brevis stabilizes the hallux during late stance by compressing the first metatarsophalangeal joint and balancing the pull of the flexor hallucis longus through the sesamoids. Small structure, big consequences. Altered mechanics at this level show up in common problems: sesamoiditis and plantar plate injury in athletes with turf toe, and progression of hallux valgus where the sesamoids drift laterally and the short flexor’s line of pull becomes a deforming force rather than a stabilizing one.
