Description
Shown in lateral orientation beneath the skin, the plantar compartment of the male foot is opened to expose the flexor hallucis brevis (musculus flexor hallucis brevis) spanning the first metatarsophalangeal joint. Its medial and lateral bellies course distal to the first metatarsal head and converge toward the base of the proximal phalanx of the hallux, typically bracketing the flexor hallucis longus tendon as it passes between the sesamoids. Adjacent osseous landmarks include the calcaneus and talus posteriorly, the midfoot tarsals, and the metatarsals and phalanges aligned distally, with plantar ligaments indicated as stabilizing bands. Focus on flexor hallucis brevis matters because first-ray mechanics often fail at the level of the sesamoid complex and plantar plate rather than at the visible bunion prominence alone. Hypertrophy, strain, or altered line of pull of the flexor hallucis brevis can contribute to hallux valgus progression, sesamoiditis, and pain after first metatarsal osteotomies when the balance between the abductor hallucis, adductor hallucis, and intrinsic flexors is disrupted. A clear look at its relationship to the flexor hallucis longus tendon and sesamoids also supports teaching of turf toe injuries, where hyperextension stresses the plantar plate and can destabilize the sesamoid apparatus.
