Description
Arising as the continuation of the femoral artery after it passes through the adductor hiatus, the popliteal artery courses inferiorly and slightly laterally across the posterior aspect of the knee, immediately deep to the popliteal vein and closely related to the tibial nerve in the popliteal fossa. A medial perspective clarifies its relationship to the medial head of gastrocnemius and the popliteus muscle, with the vessel lying on the floor of the fossa as it approaches the lower border of popliteus. Distally, the artery reaches the point of bifurcation into the anterior tibial artery, which turns anteriorly through the proximal interosseous membrane, and the tibioperoneal trunk that continues to supply the posterior tibial system. Clinically, this segment is a frequent site of aneurysm formation, and the medial view helps orient the reader to structures at risk during open repair or bypass, including the popliteal vein, tibial nerve, and genicular branches that contribute to collateral flow around the knee. The same anatomy explains why popliteal artery entrapment syndrome can occur when an aberrant gastrocnemius or fibrous band compresses the artery during plantarflexion. It matters in revascularization planning.
