Description
Running in the deepest plane of the popliteal fossa, the popliteal artery continues distally from the femoral artery after it passes through the adductor hiatus, lying posterior to the knee joint capsule and close to the popliteus muscle on the floor of the fossa. From a lateral perspective, the vessel sits posterior to the distal femur and proximal tibia, then tracks inferomedially toward its bifurcation into the anterior tibial and posterior tibial arteries at the lower border of popliteus. Companion veins are rendered in blue, with the popliteal vein typically positioned superficial to the artery and slightly lateral in the proximal fossa before crossing. Small genicular branches and arterioles may be suggested around the femoral condyle and along the tibial plateau. Deep and superficial channels are easy to separate. This angle matters when you are teaching why the popliteal artery is both protected and vulnerable: protected by surrounding muscles, but tethered proximally at the adductor hiatus and distally at the soleal arch, a setup that contributes to intimal injury in posterior knee dislocations. It is also the operative anatomy for femoropopliteal bypass and popliteal aneurysm repair, where the relationship of the artery to the popliteal vein and tibial nerve governs exposure and retraction. A tight space.
