Description
Oriented in a lateral view of the male lower limb, the sciatic nerve is seen descending in the posterior thigh and terminating in the popliteal fossa as the tibial nerve and the common fibular (peroneal) nerve. The common peroneal nerve tracks inferolaterally along the biceps femoris tendon, then winds around the neck of the fibula just distal to the lateral condyle of the tibia. At this fibular neck, it divides into the superficial fibular nerve, which continues in the lateral compartment, and the deep fibular nerve, which turns anteriorly toward the extensor compartment and dorsum of the foot. Nearby osseous landmarks include the femur, tibia, fibula, and the tarsals, metatarsals, and phalanges, with blue vascular channels coursing in parallel with the neural structures. Clinically, the fibular neck is a classic point of vulnerability because the common peroneal nerve lies superficially against bone before entering the fibular (peroneus) longus. Compression from a tight cast or brace, prolonged leg crossing, or rapid weight loss can produce a foot drop pattern via deep fibular involvement, with sensory loss over the first dorsal web space helping localize the lesion. Trauma around the lateral knee, including fibular neck fracture or knee dislocation with posterolateral corner injury, also places this nerve at risk. Short and specific. Use this lateral overview for teaching lower limb neuroanatomy in gross anatomy and physical diagnosis courses, and for illustrating the mechanism and exam findings of common peroneal neuropathy in orthopedics, sports medicine, and neurology texts. It also supports preoperative communication for lateral knee approaches where identifying the nerve proximal to its bifurcation prevents iatrogenic injury.
