Description
Arising from the common fibular (peroneal) nerve near the fibular neck, the deep fibular nerve is traced in lateral profile as it enters the anterior compartment of the leg and descends toward the dorsum of the foot. Along the proximal leg it lies deep to the lateral muscles and then courses on the anterior surface of the interosseous membrane, positioned with the anterior tibial artery and venae comitantes. Distally, the nerve passes anterior to the ankle joint between the tendons of tibialis anterior and extensor hallucis longus, continuing with the dorsalis pedis vessels on the dorsum. The tibia sits medially and the fibula laterally as constant bony landmarks. Lateral viewing makes the neurovascular pairing easy to teach because the deep fibular nerve and anterior tibial vessels travel together for most of their length, a relationship that matters in anterior compartment syndrome and during fasciotomy, where decompression must protect both structures. This is also the nerve at risk in operative exposure around the proximal fibula and in high fibular fractures, with resultant weakness of toe extension and sensory loss in the first dorsal web space.
