Description
Arising from the common fibular (peroneal) division of the sciatic nerve in the popliteal region, the deep peroneal nerve is traced from a posterior perspective as it courses from the lateral side of the knee toward the anterior compartment of the leg. The nerve hugs the proximal fibula at the level of the fibular neck, then passes deep to the extensor musculature, maintaining a close relationship to the interosseous membrane between tibia and fibula. Bony landmarks are kept in view, including the distal femur, tibial plateau, fibular head, tibial shaft, fibular shaft, and the tarsals and metatarsals distally. Posterior visualization clarifies a point that students often miss in anterior-only diagrams: the deep peroneal nerve begins posterior-lateral at the knee before it swings anteriorly to travel with the anterior tibial artery on the interosseous membrane. That trajectory matters in common fibular neuropathy at the fibular neck, where compression from tight casts, habitual leg crossing, or trauma can weaken dorsiflexion and toe extension, producing foot drop; sensory loss is classically confined to the first dorsal web space. It also frames the nerve’s proximity to the proximal tibiofibular joint and to surgical approaches around the lateral knee, where iatrogenic injury remains a real risk. Orthopaedic and sports medicine texts can pair this plate with fibular head fracture discussions, knee dislocation algorithms, and postoperative cast counseling, while neuroanatomy and regional anatomy courses can use it to teach the transition from the popliteal fossa into the anterior compartment.
