Description
Arising as the lateral terminal branch of the sciatic nerve in the popliteal fossa, the common fibular (peroneal) nerve tracks along the biceps femoris tendon toward the posterolateral knee. From a right lateral leg perspective, it is seen passing superficial to the lateral head of gastrocnemius before winding inferiorly around the fibular neck, where it lies close to bone and just distal to the lateral femoral condyle. The fibula and tibia frame the course, with the nerve positioned lateral to the proximal tibia and posterior to the anterior compartment musculature as it approaches its division into the superficial and deep fibular nerves. A tight corner. This is the classic site of vulnerability for common peroneal neuropathy. The nerve’s fixed course against the fibular neck explains why compression from leg crossing, prolonged immobilization, a tight cast or brace, or trauma around the lateral knee can produce foot drop from deep fibular involvement and sensory loss over the dorsum of the foot and lateral leg via superficial fibular fibers. For surgeons and proceduralists, the lateral view clarifies landmarks for fibular head approaches, posterolateral corner work, and safe planning around the proximal tibiofibular joint. Ideal for teaching lower-limb neuroanatomy in gross anatomy, physical therapy, and sports medicine modules, and for illustrating clinical explanations of fibular neck entrapment, peroneal palsy after knee injury, or postoperative deficits in orthopedic case reports and patient-facing materials.
