Description
Plantar soft tissues have been reflected to expose flexor hallucis brevis in the sole of a male foot, spanning the plantar aspect of the first metatarsal and dividing into medial and lateral heads that bracket the flexor hallucis longus tendon. Both heads course distally toward the base of the proximal phalanx of the hallux, each typically incorporating a sesamoid within its tendon, with the medial head lying slightly more superficial and medial than the lateral head. Adjacent plantar structures expected in this layer include the abductor hallucis along the medial border and the flexor digitorum brevis more centrally, with the first metatarsophalangeal joint and phalanges providing bony orientation. Clear tendon-bone relationships anchor the composition. Flexor hallucis brevis matters most at the first ray. Its twin sesamoid complex stabilizes the hallux on the metatarsal head during late stance, and altered mechanics here are tied to painful sesamoiditis, turf toe injuries, and postoperative imbalance after hallux valgus correction when the medial head and medial sesamoid apparatus become functionally weak or displaced. The plantar perspective also clarifies why plantar plate and sesamoid ligament integrity influences dorsiflexion at the first metatarsophalangeal joint and why direct plantar approaches require respect for the medial plantar neurovascular bundle coursing deep to abductor hallucis.
