Description
Centered in the plantar midfoot, the flexor digitorum brevis lies immediately deep to the plantar aponeurosis, its fleshy belly spanning from the medial process of the calcaneal tuberosity toward the bases of the middle phalanges of digits 2 through 5. Distally, four tendons course anteriorly and separate toward the lesser toes, positioned superficial to the long flexor tendons (flexor digitorum longus) that pass deeper toward the distal phalanges. Medially, the muscle borders the abductor hallucis; laterally it approaches the abductor digiti minimi, defining the central compartment of the sole in anatomical position. Skin texture and the plantar arch contour provide surface orientation over the intrinsic layer. Seeing flexor digitorum brevis under intact skin and superficial fascia matters because it anchors the first muscular layer that clinicians and surgeons refer to when planning plantar incisions and when interpreting focal pain under the medial calcaneus. Plantar fasciitis often localizes near the calcaneal origin of the plantar aponeurosis, but adjacent strain at the flexor digitorum brevis origin can mimic it, and this relationship is easier to teach when both structures are placed in correct depth.
