Description
Positioned in the posterior compartment view of the lower leg, the anterior tibial artery is traced after it passes through the proximal opening in the interosseous membrane, emerging from the popliteal system and entering the anterior compartment deep to the tibialis anterior. The vessel runs inferiorly along the anterior surface of the interosseous membrane, paired with venae comitantes, while the posterior tibial and fibular (peroneal) arteries remain posterior and lateral, respectively, within the deep posterior compartment. Superficial venous channels are also represented, including the great saphenous vein medially and a posterior midline course consistent with the small saphenous vein draining toward the popliteal fossa. Orientation is clear. The interosseous septum is the key landmark. This posterior perspective is a practical way to teach the transition point where the anterior tibial artery leaves the popliteal region, because that short segment at the interosseous membrane is a common site of entrapment or injury with proximal tibiofibular trauma and can be threatened during anterior compartment fasciotomy if surface landmarks are misread. Vascular surgeons also care about this course when planning bypass targets or endovascular access for critical limb ischemia, where anterior tibial patency determines the quality of dorsalis pedis runoff and distal perfusion.
