Running deep in the posterior compartment of the male leg, the posterior tibial artery descends distal to the popliteal artery and courses inferiorly along the tibia, accompanied closely by paired venae comitantes. Medially, it tracks behind the medial malleolus within the tarsal tunnel, where it lies adjacent to the tibial nerve and divides into the medial and lateral plantar arteries in the plantar foot. Perforating branches communicate laterally toward the fibular (peroneal) artery, while distal branches contribute to the calcaneal network and plantar arterial arch. A deep vessel with superficial consequences. Palpation and Doppler assessment of the posterior tibial pulse just posterior to the medial malleolus remains a routine vascular exam maneuver and a key comparison point in suspected peripheral arterial disease. In trauma and orthopedics, this artery’s position makes it vulnerable around ankle fractures and dislocations, and its tarsal tunnel relationship anchors teaching on neurovascular compression patterns.
