Seen from a posterior perspective, the reflected skin reveals the superficial trapezius spanning from the external occipital protuberance and nuchal ligament down to the thoracic spinous processes, with its fibers running laterally to the scapular spine and acromion. Inferior and lateral to it, the latissimus dorsi sweeps superolaterally from the thoracolumbar fascia and iliac crest toward the humerus, forming the posterior axillary fold at its lateral margin. Along the vertebral column, paired erector spinae columns (iliocostalis laterally, longissimus intermediate, spinalis closest to the spinous processes) occupy the paravertebral gutters, while the medial border of the scapula is bracketed by rhomboid major and minor deep to trapezius. Clear left to right symmetry helps orient the midline. Surface anatomy matters here because these planes define what you split, retract, or preserve. A midline posterior approach to the thoracic or lumbar spine takes you through trapezius and the thoracolumbar fascia into the paraspinal compartment, where multifidus and the erector spinae complex become relevant for postoperative denervation and chronic axial back pain. This view also aligns with regional anesthesia, the erector spinae plane block is performed deep to the erector spinae at the transverse processes, and correct recognition of the overlying muscle and fascia reduces the risk of an off-target injectate spread. Palpable landmarks, the scapular spine and iliac crest, anchor that mental map.
