Description
Centered along the posterior midline, the cervical vertebrae (C1 to C7) are highlighted against the rest of the vertebral column, with the spinous processes forming a stepped ridge from the atlas region superiorly down to the prominent C7 vertebra prominens at the cervicothoracic junction. Inferior to C7, the thoracic vertebrae articulate with the ribs, whose posterior angles sweep laterally from the costotransverse joints to frame the thoracic cage. The scapular girdle is not emphasized, so the bony landmarks read cleanly: cervical spine superior, thoracic spine and rib cage in the middle, pelvis and sacrum inferior. Posterior cervical bony anatomy matters when you need a reliable surface landmark. C7’s long, palpable spinous process is routinely used to count vertebral levels for physical exam, neuraxial procedures, and to orient imaging, yet it can be confused with T1 when the neck is flexed or when a patient has a prominent T1 spinous process. The posterior view also reinforces how the cervicothoracic transition changes mechanics, a common site for whiplash-associated injury and for alignment assessment in scoliosis screening or postoperative follow-up. Teaching applications include gross anatomy labs introducing regional anatomy of the vertebral column, kinesiology coursework discussing cervical and thoracic motion segments, and radiology education correlating surface landmarks with lateral radiographs, CT, or MRI level labeling. It also supports patient-facing materials explaining neck pain localization, posture, and why clinicians palpate the vertebra prominens before documenting range of motion or tenderness.
