Description
Anterior cervical vertebrae are centered in the upper spine, with the ring of C1 (atlas) sitting superior to the C2 (axis) body and framing the dens (odontoid process) in the midline. Inferior to this craniovertebral junction, the C3 to C7 vertebral bodies stack in a gentle cervical lordosis, their uncovertebral margins forming the lateral borders of the intervertebral spaces. The thoracic vertebrae and posterior rib cage continue caudally, and the iliac crests appear at the inferior extent as pelvic landmarks for overall spinal alignment. Orientation is strictly anterior, so the vertebral bodies dominate while spinous processes remain posterior and less emphasized. C1 to C2 anatomy matters because the atlanto-axial joint accounts for a large share of cervical rotation, and the dens acts as the pivot that must remain constrained by the transverse ligament of the atlas. A subtle change in the relationship between the dens and anterior arch of C1 is the difference between normal motion and atlanto-axial instability, a concern in trauma, rheumatoid arthritis, and congenital syndromes. This angle also supports teaching the biomechanical transition from the craniovertebral junction to the lower cervical spine, where uncovertebral joints influence foraminal narrowing and radicular symptoms.
