Description
Arising bilaterally from the thyrocervical trunks, the inferior thyroid arteries course medially and superiorly across the prevertebral fascia toward the posterior aspect of the thyroid lobes. From a posterior perspective, each vessel approaches the gland near the inferior pole and then distributes ascending branches along the posteromedial surface, deep to the thyroid capsule and close to the attachment of the isthmus. The paired lobes flank the midline isthmus, and the parathyroid glands appear as small ovoid bodies on the posterior thyroid surface, typically clustered near the upper and lower thirds of each lobe. Fine arterioles radiate over the gland’s lobulated contour. This angle matters because the inferior thyroid artery forms the dominant arterial supply to the parathyroid glands and runs in the same surgical neighborhood as the recurrent laryngeal nerve in the tracheoesophageal groove. Ligation too close to the thyroid capsule during thyroidectomy can jeopardize parathyroid perfusion and raise the risk of postoperative hypocalcemia, while dissection around the artery is also where recurrent laryngeal nerve injury occurs. Small variants, including asymmetric caliber and variable branching before gland entry, are easier to teach from the posterior surface where those relationships are most consistent. Endocrine surgery teaching sessions use this view to explain safe planes for capsular dissection, preservation of parathyroid vascular pedicles, and where to look for the recurrent laryngeal nerve relative to the inferior thyroid artery.
