Description
Oriented from a posterior perspective in an adult male, the paired thyroid lobes flank the upper trachea and meet at the midline isthmus, all contained by the fibrous capsule. Arterial inflow is traced along the posterolateral surfaces of the gland, with the superior thyroid artery descending from the external carotid system toward the superior poles and the inferior thyroid artery approaching the inferior poles from the thyrocervical trunk. Venous outflow is organized into superior, middle, and inferior thyroid veins, shown coursing from the gland toward the internal jugular and brachiocephalic venous pathways. Small structures sit immediately posterior to the lobes. The parathyroid glands lie on the posterior capsule, close to the terminal branches of the inferior thyroid artery. Posterior thyroid vasculature matters because it is where endocrine anatomy and operative risk intersect. During thyroidectomy and central neck dissection, surgeons often encounter the inferior thyroid artery near the expected course of the recurrent laryngeal nerve in the tracheoesophageal groove, and the relationship between these structures drives decisions about ligation to reduce nerve injury. Preservation of parathyroid perfusion is equally tied to this view, since the inferior thyroid artery commonly provides the dominant blood supply to the parathyroid glands, and devascularization can precipitate postoperative hypocalcemia.
