Description

Centered low in the anterior neck, the paired thyroid lobes flank the trachea and meet across the midline via the isthmus, all enclosed by the thyroid capsule. Superiorly, branches consistent with the superior thyroid arteries descend toward the superior poles, while inferior thyroid arteries approach the gland from a more caudal, lateral course toward the posteroinferior aspect of each lobe. A dense venous plexus overlies the lobes, with superior and middle thyroid veins draining superolaterally and inferior thyroid veins converging inferiorly toward the brachiocephalic venous system. The vascular crowding is the point. Anterior vascular anatomy drives decision-making in thyroid surgery because small-caliber vessels cross the operative field early in the approach, before the recurrent laryngeal nerve and parathyroid glands are encountered posteromedially. Ligation of the superior thyroid vascular pedicle close to the gland reduces risk to the external branch of the superior laryngeal nerve, while attention to the inferior thyroid veins near the lower pole helps control bleeding during mobilization of the isthmus and inferior pole. Hypervascular states, including Graves disease and some thyroiditis patterns, make these relationships clinically loud. Endocrine anatomy teaching benefits from this plate when introducing the thyroid’s high-flow physiology and the named arterial and venous channels that students are expected to trace in dissection and angiographic correlation.
