Description
Posteriorly, the thyroid gland appears as paired lateral lobes joined by a midline isthmus, all invested by a thin capsule. Blue vascular channels branch across the posterior surfaces of each lobe and converge toward the posteromedial borders, consistent with the dense intrathyroidal capillary bed and venous plexus. Four small, ovoid parathyroid glands sit on the posterior aspect of the lobes, typically two superior and two inferior, closely applied to the capsule near the region where the inferior thyroid vessels enter. Relationships are tight. That posterior relationship between the thyroid capsule, parathyroid glands, and the dominant vascular pedicles is the surgical problem in thyroidectomy and parathyroid exploration. Ligation of the inferior thyroid artery too proximally can jeopardize parathyroid perfusion and set up postoperative hypocalcemia, while venous bleeding from the capsular plexus can be disproportionate to the gland’s size and complicate dissection in the tracheoesophageal groove. This view also supports teaching why the thyroid’s rich vasculature underlies brisk enhancement on contrast CT and the vascularity seen on Doppler in Graves disease. Use this illustration in endocrine anatomy labs, operative anatomy teaching for thyroid lobectomy and central neck dissection, and figure panels for chapters on thyroid and parathyroid surgery where posterior capsular dissection and vascular control must be explained cleanly.
