Description
Posteriorly oriented thyroid anatomy is centered on the paired lobes and intervening isthmus, with the inferior thyroid veins emerging from the inferomedial aspects of the gland and descending toward the superior mediastinum. From this angle, the venous channels can be followed as they converge into short trunks that lie anterior to the pretracheal fascia and course inferiorly toward the brachiocephalic veins, while smaller venules perforate the thyroid capsule along the posterior surface. The parathyroid glands appear as small ovoid bodies on the posterior thyroid lobes, typically near the junction of the superior and middle thirds, in close relationship to the venous plexus and capsular plane. A posterior perspective matters because the inferior thyroid veins are a common source of troublesome bleeding during low cervical exposure, thyroidectomy, and tracheostomy, where they can be thin-walled, variable, and tethered near the thoracic inlet. Their proximity to the trachea and the pretracheal venous plexus explains why venous injury can occur even when the dissection stays close to the gland, and why meticulous ligation near the inferior pole reduces obscuring hemorrhage.
