Description
Prominent in the anterior neck, the right and left lobes of the thyroid gland flank the midline isthmus, all enclosed by the thyroid capsule and traced by a superficial venous network of venules and larger thyroid veins. Short middle thyroid veins emerge from the anterolateral surface of each lobe and course laterally, crossing the capsule toward the internal jugular vein within the carotid sheath. Their relationship is tight: the veins sit lateral to the gland but medial to the sternocleidomastoid, and they lie anterior to deeper pretracheal fascia and tracheal rings. Small caliber, short run. This anterior view matters in thyroidectomy because the middle thyroid vein often represents the first substantial lateral venous tether between the thyroid lobe and the internal jugular, and early control improves mobilization of the gland. Avulsion at the jugular junction can produce brisk bleeding and an obscured field, a familiar problem when elevating the lobe laterally to identify the recurrent laryngeal nerve and preserve the parathyroid glands. For central line placement and neck dissection, appreciating the proximity of these tributaries to the carotid sheath helps explain unexpected venous injury during dissection along the jugular.
