Description
Positioned deep in the posterior lumbar region, the intertransversarii laterales lumborum appear as short muscle slips bridging adjacent transverse processes from the upper to lower lumbar vertebrae, immediately lateral to the zygapophysial joints and medial to the quadratus lumborum. Overlying layers in this posterior perspective include the thoracolumbar fascia and the longitudinal columns of the erector spinae, while more superficial coverage suggests the broad contour of latissimus dorsi inferiorly and trapezius superiorly. Spinous processes form the midline reference, with paired paraspinal masses flanking them from thoracic levels down toward the posterior iliac crests and gluteal region. Small muscles, big landmarks. Because these intertransversarii are easily overlooked, a posterior view that anchors them to specific lumbar transverse processes helps clarify segmental stabilization concepts at the lumbar motion segments. Clinically, this matters when interpreting focal paraspinal tenderness or postoperative anatomy after posterior lumbar approaches, where intermuscular planes between multifidus, longissimus, and adjacent deep stabilizers can be developed for exposure of the facet complex and transverse processes. Their proximity to the dorsal rami and the attachments of thoracolumbar fascia also makes them relevant when teaching mechanisms of mechanical low back pain and the anatomy referenced during medial branch blocks.
